You can also pay your bill through our patient portal or by calling our Business Office at 425.485.3955.
We participate with most major health plans ( click here for a full list of our health plans).
Please contact your health plan directly if you have questions regarding eligibility or benefits as this is a contract between you and your health plan.
If you are an established patient without insurance coverage or without proof of insurance, you will be expected to pay a $125 deposit in advance of your visit. This amount may not be the total charges associated with your visit. If you have lab, x-rays, additional tests, or a high level of service, those charges will be billed to you.
You will receive a paper statement each month for any balance due on your account. This includes amounts for services billed to your insurance which have not been paid after 90 days from the date of service.
Co-pays, co-insurance and any patient responsibility balance are due at the time of service. For your convenience, we accept cash, debit card, Discover, Visa or MasterCard.
Motor vehicle and third party injury claims are your financial responsibility. We will provide documentation for you to submit as evidence of services rendered. We do not do third party billing.
We understand there are times you may not be able to pay your bill in full. In those instances, it is your responsibility to call our Business Office to make special payment arrangements.
Delinquent accounts are turned over to a collection agency following non-payment of a second statement.
